On 19 May 2015, the Multi-Stroke Ltd worker was tasked with setting up hot forging machinery in order to use for the next job, Wolverhampton Magistrates’ Court heard.
But, his feet slipped and his left hand was crushed between the dies. He lost his thumb, index finger and middle finger.
A Health & Safety Executive (HSE) investigation found that the company failed to implement a safe system of work for the work place activity. The firm also failed to adequately manage the risks associated with using the machinery.
HSE inspector Jonathan Jones said: “The company failed to implement a safe system of work which ultimately led to this serious incident occurring. Duty holders have the responsibility to effectively plan this work and manage the risks associated with working with this type of heavy machinery.”
Multi-Stroke Ltd of Locarno Road, Tipton, West Midlands pleaded guilty to breaching Section 2 (1) of the Health and Safety at Work Act 1974. It was fined £40,000 and ordered to pay costs of £2,270.56.
